Remember that...

You can present a claim online, by fax or at Citizen Help and Information Office (OAC) registers, provided that: 

  • You have already lodged a claim with the company beforehand, as explained in the Requirements for submitting a claim section.
  • You produce a copy of the documents you have on the issue concerned.
  • You fill in the claim/complaint form (available online or in PDF format, if the establishment has not provided you with one).
  • You are the holder of the invoice, contract or service you are making your complaint or claim against and you present a copy of your ID document. If the invoice is not in your name, we will need the holder's express authorisation and a photocopy of both ID documents.
