Barcelona City Council and the Department of Health at the Generalitat have reached an agreement for developing a care model that will enable the integration of the social and health-care services with the help of IT systems.

Description

The programme for a comprehensive approach to healthy active ageing in Barcelona’s neighbourhoods has been created to enable social and health-care data exchanges, based on interoperability, and allowing exchanges of social and health-care information on people over the age of 65 who require home care, both in health (ATDOM) and in social services (SAD) and also for patients with complex chronic illnesses (PCC), people with advanced-stage chronic illnesses (MACA), and people with a telecare service.
